Mental Health Case Management:
Three agency social workers provide a range
of case management and supportive services
to individuals and families experiencing
problems or difficulties related to mental
illness. Services that are available to
support individuals and families include
case management services, pre-petition screening
inpatient admission, civil commitment proceedings,
discharge planning, placement, information
and referral.
Case
management functions include providing referrals
to the community mental health center, community
outreach programs, day treatment groups,
local social club/drop in program, vocational
programs, residential treatment facilities,
board and lodging facilities, money for
housing support and assistance from local
food shelves. Staff serves 74 persons through
general case management services and 72
clients who experience severe and persistent
mental illnesses such as schizophrenia,
affective disorders, personality disorders
and/or chemical dependency problems. They
work closely with two other case managers,
employed by the community mental health
center, who actively serve 60 additional
individuals experiencing a major mental
illness.
In
2003, there were 30 pre-petition screening
interviews, and 21 civil commitments. This
program works closely with the Becker, Clay,
Otter Tail and Wilkin County Adult Mental
Health Initiative, comprised of county,
community provider and regional treatment
center staff, consumers and family members.
