Disabilities and Related Conditions:
Six agency social workers provide Developmental Disabilities Case Management services to children and adults across a variety of settings- within the family home, individual apartments, adult foster care or Intermediate Care Facilities- DD. Case management functions include eligibility determination, information & referral, screening, service planning & monitoring, and funding authorization. The goals of case management services are to promote inclusion, self-sufficiency & the highest level of community independence; support social relationships & natural supports, and to maintain a balance between safety and opportunities. Supports are also in place to protect people with developmental disabilities from abuse, neglect or exploitation, to support & preserve families, maximize functional independence and promote community integration Some of the services available to consumers are: respite services, adaptive equipment, IEP & transition planning, vocational supports, semi-independent living skills training, supervised living services, adult foster care, and acting as Guardian Representatives for State Wards.
